Colours used are orange, purple, blue and white. The name "Andrew" is cut out of paper, each letter alternating between purple and red, and is affixed along the centre of the upper edge of the backing paper.

Figure has orange and aquamarine-coloured pieces for the torso and legs, blue circles for arms, a white circle for his head, and black circles for the eyes, hands and feet. Small strips of black are applied to the top of the head for hair, extending past the upper edge of the backing paper. The temple (part a) has three sides closed with glass and one shaped arch opening surrounded by a fence along both sides which leave an opening in front of the shaped arch opening and on its opposite side. The roof (part b) tapers upward along all four sides to a smaller square top at the centre. There is a bronze bell at each of the four corners of the roof (part b).
Skin formed into the shape of a hollow shrunken head (part a) that is stitched at the back, with long black coarse hair still attached at top. Eyebrows and eyelashes also have trimmed coarse black hair attached. The 'head' is housed in a hexagonal-shaped box (part b) that resembles a small coffin.
